Packet 3B - Responding to a petition for custody & parenting time - If you are unmarried and the responding to a summons and petition (court case) for custody and to establish for the first time custody, parenting time, and/or support for a child under 18 years old, print and complete this packet of forms. Packet 4A - Order for Temporary Custody - If you have reached an agreement or have been awarded temporary custody pending the outcome of your case, print and complete this form.

Packet 4C – Stipulation (Agreement) to Modify Judgment – If both parents agree to specific changes and want to modify their Judgment, they may use one of the forms below
Packet 5 - Filing to modify an existing judgment regarding any issue - If you wish to file a motion to modify a judgment as to any issue including custody, parenting time and/or child support, print and complete this packet of forms. PLEASE CAREFULLY READ THE INSTRUCTIONS.
Packet 6 - Filing to modify an existing judgment regarding all issues except custody, parenting time or support - If you wish to file a motion to modify a judgment as to any issue except custody, parenting time and/or support, print and complete this packet.
Packet 7 - Supplemental Judgment Modifying Judgment – This form is used to modify an existing judgment after an Evidentiary Hearing has been held.
If both spouses want to file for dissolution (divorce) together and reach complete agreement regarding terms they request in the decree, one of the following packets may be appropriate. Filing together as co-petitioners eliminates certain formalities such as needing to arrange to have documents served on the other spouse.

Packet - Request for Status Quo Order POST-JUDGMENT - You may use this packet to request an order keeping the child(ren)'s daily routine from changing after a judgment establishing custody/parenting time has been entered.
