Adoption

Court Staff are not permitted to give legal advice or to advise you about which forms need to be filed. Parties should contact an attorney or research the issue themselves.

Any person may petition the Circuit Court for leave to adopt another person and if desired, for a change of the other person's name.  One petitioner, the child, or one birth parent must be a resident of this state for at least six months.

Adoption proceedings can be complicated. It is recommended that you contact an attorney for advice.

DOCUMENTS (sometimes these forms are filed later in the adoption process or are filed as one document under the title of Petition for Adoption)

  • Petition for Adoption
  • Vital Records form and fee
  • Financial Disclosure Statement
  • Consent of Biological Parents/Consent of Child (if 14 years or older)
  • Certificate of Residency
  • Original or certified copy of foreign adoption
  • Adoption/Placement Report (only for adoptions of minors)

OR

  • Department of Human Services, Child Welfare Office (DHS) waiver
  • Medical Report (except for adoption by step-parent or adoption of an adult)

  • Motion and Order for Appointment of Guardian
  • Motion and Order for Publication
  • Motion and Order to Show Cause

 


OREGON'S VOLUNTARY ADOPTION REGISTRY

The Voluntary Adoption Registry was created in 1983 to assist adopters, birth parents, biological siblings and other eligible persons in learning more about an adoption that was finalized in Oregon. The registry creates a method of communicating information while preserving confidentiality and respecting the right to privacy. The Adoption Services Unit for the State of Oregon is responsible for maintaining records on all adoptions going back to the early 1920's.
