Online Jury Response Form
The Web Jury Response Form has four parts:
- Information required from the Jury Summons you received;
- Fields to enter and to confirm your e-mail address;
- Your response
The first section must contain the same information as that located on your jury summons. Please ensure that this data matches. If information is missing, you will be asked to supply the missing information before the form can be submitted.
The e-mail address is used only to respond to your request. The Court does not share, sell, lease or rent your e-mail address. The e-mail address will not be used for any purpose other than correspondence relating to the jury summons which you have received.
If you do not want to provide a valid e-mail address where you can be notified, you may not use the form. Please fill out and mail the jury response card which you received with your jury summons .
The response area of the form allows you to indicate your preference for jury duty. Please check the appropriate box and, if necessary, a comment if you believe additional information is needed by the Jury Coordinator.
Within a few minutes after you have filled out the form and clicked on the 'Submit' button at the bottom of the form, you will receive a confirmation message at the e-mail address you provided.
Please click the 'Submit' button only one time.
When you are ready, you may fill out the Jury Response form.
