The Office of State Court Administrator has prepared instructions and forms for persons seeking a restraining order under the Elderly Persons and Persons With Disabilities Abuse Prevention Act (ORS 124.005 - 124.040). Litigants should check with their local courts and facilitators to find out whether the court in their county will accept the use of these forms, and for more detailed information about local rules and procedures. Once completed, the forms should be filed with the clerk of your local court.
If your case involves complicated legal issues, you may seriously harm your legal rights if you use these forms without consulting with a lawyer. You are strongly urged to talk to a lawyer. You may find a lawyer by calling the Oregon State Bar's Lawyer Referral Service and Modest Means program at 1.800.452.7636. You may also contact your local legal aid office. For more information about legal services and information, visit the Oregon State Bar's website at http://www.osbar.org/.
Translations of the final version of the Elderly Persons and Persons With Disabilities Abuse Prevention Act forms are available in Spanish, Russian, Vietnamese, and Korean. NOTE: Forms must be translated into English before filing with the court. Please contact your local court if you need an interpreter.
