For more information about applying for, modifying or renewing a FAPA restraining order, you may contact your local court or facilitator.   Once completed, the forms should be filed with the clerk of your local court.

Translations of the final version of the FAPA forms are available in Spanish, Russian, Vietnamese, and Korean.  NOTE: Forms must be translated into English before filing with the court. Please contact your local court if you need an interpreter.

Revised FAPA Benchguide 2012

The FAPA Benchbook has been revised and updated in 2012 by the FAPA Benchbook Revision Workgroup of the State Family Law Advisory Committee’s (SFLAC) Domestic Violence Subcommittee, comprised of Amber Frye, Legal Aid Services of Oregon, Rebecca Orf, Oregon Judicial Department, and Robin Selig, Oregon Law Center.  The Honorable Paula J. Brownhill, the Honorable Maureen McKnight, the Honorable Lorenzo Mejia, and the SFLAC Domestic Violence Subcommittee provided additional input and review.  The work of contributors to previous versions of the Benchbook is acknowledged.
